The rule requires only completed parts to be maintained, and (almost always) only 2, 3 or 4 parts will be required for any facility. Therefore, each part has its own "form page" with user-friendly fillable fields and guidance. These are accessible from the complete form (above) and from the Forms Page (that led you here). The individual user-friendly form pages include additional guidance.
