Instructions (guidance, really) and tables on pages i and ii may prove useful
The several mechanisms (Parts A through O) (with links to single-form-part versions)
The individual user-friendly form pages include additional guidance
Certification of Financial Responsibility (Part P) (with a link to the single-form-part version)
The rules require only completed parts to be maintained, and (almost always) only 2, 3 or 4 parts will be required for any facility. Therefore, each part has its own "form page" with user-friendly fillable fields and guidance. These are accessible from the complete form (above) and from the Storage Tank Financial Assurance Forms Page (that may have led you here). See also the Form Guidance (FAQ) page. Note: the formally adopted form is available on the Secretary of State's website [Department of State link].
What to Do with Completed Forms
An owner or operator must maintain financial responsibility records (the completed form parts with required supporting documentation) at the facility or at their business office
If not kept at the facility, they shall be made available at the facility or another agreed upon location upon five business days notice by a department or county inspector
