Phosphogypsum Stack System Closure, Water Management and Long-Term Care Cost Estimate

PLEASE NOTE: Forms for facilities that discharge to surface waters must be completed, printed, hard-copy signed, and then mailed, along with any applicable attachments, to the applicable DEP office as outlined in the instructions document below. 
A form instructions document is available below and contains important information regarding submitting these forms.

  • All permit application forms associated with phosphate management facilities should be submitted to either the Northeast District Office for facilities within the boundaries of the Northeast District or to the Phosphate Management program in Tampa for all other facilities. 
  • Note, an electronic copy of the form will constitute the multiple additional copies that are required by Rule 62-620.310(3), F.A.C for facilities that discharge to surface water.  To email the electronic copy please click the applicable DEP Office below.
  • Forms for facilities that do not discharge to surface waters may be either mailed to the appropriate DEP Office or submitted via email.
