Phosphogypsum Stack System Construction/Operation Permit Application

PLEASE NOTE: Forms for facilities that discharge to surface waters must be completed, printed, hard-copy signed, and then mailed, along with any applicable attachments, to the applicable DEP office as outlined in the instructions document below. 

All permit application forms associated with phosphate management facilities should be submitted to either the Northeast District Office (for facilities within the boundaries of the Northeast District) or to the Phosphate Management program in Tampa (for all other facilities). 

Submit signed, hard-copy forms to the appropriate office at the following address:

Note: an electronic copy of the form will constitute the multiple additional copies that are required by Rule 62-620.310(3), F.A.C for facilities that discharge to surface water. 

Forms for facilities that do not discharge to surface waters may be either mailed to the appropriate DEP Office or submitted via the Phosphate Management program email address.
