Application to Discharge Wastewater from Concentrated Animal Feeding Operations and Aquatic Animal Production Facilities - Form 2B

62-620.910(3), Application to Discharge Wastewater from Concentrated Animal Feeding Operations and Aquatic Animal Production Facilities - Form 2B is a form that is used for surface water discharge (NPDES) facilities.  NPDES forms must be submitted by mail.  However, an electronic copy of the form will constitute the multiple additional copies that are required by Rule 62-620.310(3), F.A.C. 

This form should be submitted, along with any other applicable forms and attachments, to the DEP Office for the county in which the industrial wastewater facility or activity is located.
