Welcome to the Department of Environmental Protection’s (DEP) industrial wastewater forms website. DEP has implemented a paperless document submittal process that will allow for the electronic submission of certain industrial wastewater forms, including plans and other documents associated with the forms. In order to complete the following DEP electronic forms, you must have Adobe Reader 8.0 or higher. We suggest that you download free Adobe Reader 11.
An electronic copy will constitute the multiple copies that are required by Rule 62-620.310(3), F.A.C., therefore there is no need to submit multiple hard copies.
Please note that Forms 62-620.910(1), (2), (3), (5), (7), (8), (9), (11), (12), (13), 62-621.300(3)(b), (8)(f) and (g) cannot be submitted electronically by facilities that discharge to surface waters under the National Pollutant Discharge Elimination System (NPDES).
