Wastewater Facility or Activity Permit Application - General Information - Form 1

62-620.910(1), Wastewater Facility or Activity Permit Application - General Information - Form 1 is a form that can be used by both surface and ground water discharge facilities. 

For surface-water discharge (NPDES) facilities, this form must be submitted by mail.  However, an electronic copy of the form will constitute the multiple additional copies that are required by Rule 62-620.310(3), F.A.C. 

For groundwater discharge (non-NPDES) facilities, this form can be submitted either electronically or by mail. 

This form should be submitted, along with any other applicable forms and attachments, to the DEP Office for the county in which the industrial wastewater facility or activity is located.

Please note that permitting of certain industrial wastewater facilities that discharge to groundwater only has been delegated to Hillsborough County.  Please contact the Hillsborough County Environmental Protection Commission (EPC) for the appropriate forms and permitting procedures
