62-621.500(1)(b), Request for Coverage Under the Generic Permit for Discharges from Fresh Citrus Fruit Packinghouses to Percolation Ponds is a form that is used for ground water discharge (non-NPDES) facilities. This form can be submitted either electronically or by mail.
This form should be submitted, along with any other applicable forms and attachments, to the DEP Office for the county in which the industrial wastewater facility or activity is located.

Please note that permitting of certain non-NDPES industrial wastewater facilities that discharge to groundwater only has been delegated to Hillsborough County. Please contact the Hillsborough County Environmental Protection Commission (EPC) for the appropriate forms and permitting procedures.
