62-621.300(2)(b), Notice of Intent to Use the Generic Permit for Discharge of Ground Water from Dewatering Operations is a form that is used for surface water discharge (NPDES) facilities. If this form is being submitted by mail, please complete form 62-621.300(2)(b) and submit to the appropriate DEP Office for the county in which the industrial wastewater facility or activity is located. Note, an electronic copy of the form will constitute the multiple additional copies that are required by Rule 62-620.310(3), F.A.C.
