SRF Request for Reimbursement Summary Form

State Revolving Fund loan agreements specify that disbursement requests must be accompanied by sufficiently itemized summaries of the materials, labor, or services to identify the nature of the work performed; the cost or charges for such work; and the person providing the service or performing the work, and proof of payment. Here is an example of a summary that sponsors can use for this purpose. All disbursement requests will need to include a summary of invoices submitted for reimbursement.
