Domestic Wastewater Facilities submitting non-NPDES forms electronically should email the completed form to the appropriate district office listed below. Facilities submitting Form 62-610.300(4)(a)4, Pathogen Monitoring Report, should also use these email addresses.

To determine the appropriate District Office, identify the county in which the domestic wastewater facility or activity is located then consult the District boundaries map shown on the right.
The department, (DEP) revised the Annual Reuse Report form (DEP Form 62-610.300(3)(a)2) and the reporting requirements in Chapter 62-610, F.A.C., effective Aug. 8, 2021.
In accordance with section 62-610.870(3)(a), F.A.C., and the Annual Reuse Report form instructions, the report form must be completed online and additional information such as inventories must be electronically submitted through the DEP Business Portal as PDF or Word documents. Submission through the portal satisfies the requirement to submit the information to both the Tallahassee DEP Wastewater Management Program office and the appropriate DEP district office.
Please note: you are also required to submit a copy of the report separately to the appropriate Water Management District. The DEP Business Portal can generate a downloadable Annual Reuse Report in PDF format that you can submit to that district.
